compassionate healthcare for your family

Housing White

community-based health services

Designed with all of the needs of a patient in mind, West Valley Family Care provides medical care, counseling, assistance with medication management, and support services as needed.

Compassionate staff work with children and adults to overcome the challenges they face, whether it be ADHD, depression, anxiety disorders, relationship issues, substance use, or trauma-related issues, such as abuse, neglect, or domestic violence.

All of these services are provided by licensed therapists, case managers, psychiatrists, nurse practitioners, and primary care physicians who care deeply for their work and patients. This integrated care approach allows an individual to obtain all of the care they need to address their challenges in one convenient location – dramatically improving their health and wellness.

Clients can remain in services as long as they maintain their eligibility status, and until all identified treatment goals are achieved, which is determined through collaboration with their treatment team.

If you are a new client or haven’t been seen in over a year, please call 480-969-6955 (East Valley) or 623-934-1991 (West Valley) to schedule an intake appointment.
